Enrollment Procedures

A registration packet can be obtained from any of the school offices.

The following information is required before your child can be enrolled in the school district:

   1. A fully completed Student Registration Form, with parent/guardian signature.

   2. PIN# (Property Identification Number) which can be found on your tax bill or by calling your local tax collector or the County Tax Assessor's Office at 570.420.3412.

   3. Signed Pennsylvania Weapons document (to be signed in the presence of a school representative).

   4. Signed Parental Consent for Use of Student Photograph, Videotape, or Other Images Document.

   5. Fully completed Release of Records form (including address of former school).

   6. Fully completed Emergency Card (must have two local emergency contact phone numbers).

   7. Original birth certificate or passport.

   8. Fully completed Transportation Registration (this will be completed by the office on the elementary/intermediate level).

   9. Two (2) official Proofs of Residency, e.g.: current utility bills, lease or deed, receipt for payment of municipal and/or school district taxes, receipt for payment of earned income tax from Berkheimer Associates, selective service card, voter registration card, registration of motor vehicle card, driver's license, and/or automobile insurance card.

  10. Up-to-date Immunization records.

  11. Social Security Number for both student and parent(s).

  12. Most recent report card or grades to date/transcripts from previous school of attendance.

  13. Educational testing results/scores (i.e. CAT, PSSA, IOWA, etc.)

  14. IEP's, CER's, Psychological/Psychiatric reports.

  15. Any specially designed individual school/guidance forms (i.e. Free/Reduced Lunch Form, Student History for Kindergarten use only, Custody Papers and Forms, Alternate Bus Stop Form, etc.)

Kindergarten Requirements for Incoming Students

Primary Elementary Level (K-2):
When registration packet is complete, please call your school office for an appointment.

Intermediate Elementary Level (3-4):
When all of the above information is complete, please call the Guidance Office to schedule a registration appointment.

Middle, Junior High and High School Level (5-12):
When all of the above information is completed, please schedule an appointment with the Guidance Office for review. You may also schedule a meeting with the guidance counselor to schedule your child's classes. Read through the Program of Studies and the Educational Planning Guide provided to you in your packet to assist you in choosing your preferred courses prior to your scheduled meeting with the counselor. This will help you in selecting the best course of study to meet your son's/daughter's needs.

Notes for special education transfers

When a special education student transfers to the Stroudsburg Area School District from another Pennsylvania school district, our district will maintain the IEP from the previous Pennsylvania school district pending preparation and implementation of a new IEP.

When a special education student transfers to the Stroudsburg Area School District from another state, our district will review the student's IEP and any other relevant information as soon as possible and will convene a meeting with the parents to discuss the student's program.
